Although not pictured, the Leviton model "1755" means the combined 3-switch in the nice Decora style. "A" means Almond, "I" means Ivory, "W" means white, etc. The "004" seems to indicate slight variations such as whether it comes with a single-gang wall plate or not (this one does not -- just the switch). I purchased 4 "004-1755-a" units from Amazon and am very happy with them. I searched a long time for a nice set of combined 3-switches, and in Almond. They seem to be out of stock everywhere, but Amazon had them. Anything 18 or less seems to be a good price from my shopping around. White are a little easier to come by in stores (Home Depot & OSH) but Almond have been next to impossible to find in stores. OSH had them, but is out of stock at all locations withing driving distance of my home. Lowes carries a 3-switch, but it is not the nice "decora" style and seems to be much more of a utility switch than something I'd want inside my house. I really like the "Decora" style rocker switches. They match other decora style rocker switches that are easy to find at any store. Watch out for some stores that tag some 2-switches as "Decora" on the price sticker but area actually are not the nicer Decora switches (Lowes) in some cases. The box will actually say "Decora" and probably shouldn't say only "Cooper".
These 004-1755-a units from Amazon (at least the ones I received) were exactly the Decora almond 3-switches I had wanted. I've used one so far and it works exactly as expected.
Note that the actual unit designs vary slightly among the 4 I received (which is fine with me). There seem to be 2 designs for the *back* of the unit, one with screw terminals (and optional push-in) and the other seems to be only push-in (no screws).
With the 3-switch, you only have 1 "hot" input and all 3 operate from the same hot input. The traditional 2-switch rockers typically have a tab you can break and 2 hot inputs should you have two separate power sources. Most people don't need that capability though. I only needed it in a unique case.
Overall, I am completely satisfied, as far as one could be with wall switches. I simply got what I wanted, a 1755-A (as pictured on Leviton's website).
